对于某些未经治疗的扁桃体原发性弥漫大B细胞淋巴瘤（DLBCL），长期无瘤生存是很有可能的。作者报道9例未经治疗而长期无瘤生存的扁桃体DLBCL。9例患者均为儿童或年轻人（男性4例，女性5例；年龄：6-38岁，平均年龄25岁），临床表现为扁桃体明显肿大或乳头状突起的赘生物。

扁桃体切除和活检显示扁桃体局部结构破坏，伴大的淋巴细胞弥漫浸润；大细胞表达CD20，Ki-67增殖指数50%。所有病例CD5和Epstein-Barr病毒编码的RNA均阴性，且不存在t(14;18)易位。除病例2和病例4外，所有病例均显示单克隆性免疫球蛋白基因重排。由于各种原因，所有患者在扁桃体切除术后均未行放疗和化疗，但是定期的影像学及临床评估显示9例患者病情均未见进展，也没有因淋巴瘤死亡（平均随访时间：40个月）。

总之，原发性扁桃体DLBCL并不总是表现为恶性生物学行为，一些未经放疗或化疗的患者也可以长期无瘤生存。根据这9例病例，作者推断儿童或年轻人扁桃体原发性DLBCL患者如果具有以下特点：病程短、形态学为早期病变、Epstein-Barr病毒编码的RNA阴性、不存在t(14,18)易位和未累及任何其它部位，经过仔细的临床评估后，可以在完整的扁桃体切除术后进行密切的随访和观察即可。

Tonsillectomyandbiopsiesindicatedpartialstructuraldestructionofthetonsilswithdiffuseinfiltrationoflargelymphoidcells.ThelargecellsexpressedCD20withaKi-67proliferativeindex50%.AllsampleswerenegativeforCD5,Epstein-Barrvirus-encodedRNA,andt(14;18)translocation.Exceptforcases2and4,allsamplesshowedmonoclonalimmunoglobulingenerearrangements.Althoughchemotherapyandradiotherapyhadnotbeenadministeredaftertonsillectomy(forvariousreasons),periodicimagingandclinicalevaluationshowedthatnoneofthe9patientsdevelopedordiedoflymphoma(medianfollow-up:40mo).

Inconclusion,primarytonsilDLBCLdoesnotalwaysbehaveinamalignantmanner,andsomepatientscanachievelong-termtumor-freesurvivalwithoutchemotherapyorradiotherapy.Onthebasisofthiscaseseries,weconcludedthatclosefollow-upandobservationmightbepossibleforpediatricandyoung-adultpatientswhohaveundergone
